Jake Monaco
Senior Occupational Therapist and HCP Team Leader
Jake has extensive experience in home modifications, assessments and assistive technology. Jake works with young people and adults with disabilities to improve their independence and confidence to live meaningful lives. He is also the Team Leader for Home Care Package (HCP), for people over 65 years, assisting them to remain safe and independent in their own homes.
Lucy Harkness
Senior Occupational Therapist and NDIS Clinical Lead
Lucy works in regional South Australia across Murray Bridge and the Fleurieu Peninsula. She primarily works with children and young adults with Autism Spectrum Disorder and Intellectual Disability, and adults with chronic neurological conditions, assisting them to achieve their goals. Lucy supports people in acquiring suitable assistive technologies, minor and complex home modifications.
Emma Pavia
Senior Paediatric Occupational Therapist
Emma works in early intervention, with infants, children and teenagers with various disabilities, including Autism Spectrum Disorder, Attention Deficit Hyperactivity Disorder, Global Developmental Delay, Intellectual Disability, and trauma. Emma provides intervention, assessment and ongoing supports to help participants to achieve their goals.
Melanie Dinning
Senior Occupational Therapist
Melanine has extensive experience in community based occupational therapy with adults including assessment and interventions. She supports people to remain safe and independent both at home and in the community. Melanie has additional experience in neurological conditions including Stroke, Motor Neurone Disease, Parkinsons and Multiple Sclerosis.
Kirsten Read
Senior Occupational Therapist
Kirsten has extensive experience working with adults and older adults with mental health disabilities. She provides intervention, assessments and supports people in acquiring suitable assistive technologies to remain independent and achieve their goals.
Chloe Perks
Senior Paediatric Occupational Therapist
Chloe works in early intervention, paediatrics (infants to teenagers) and younger adults. She has experience with various disabilities including Autism Spectrum Disorder and Attention Deficit Disorder. Chloe provides intervention, assessments and outreach to Port Lincoln every month, supporting participants to achieve their goals.
Ryan Thompson
Occupational Therapist
Ryan has experience in community based occupational therapy with expertise in home modifications, assistive technology and home based interventions. Ryan’s special interest areas include working with school aged children through to adults to assist them in improving their participation and independence in daily life.

Shannon Calderbank
Senior Physiotherapist and HCP Team Leader
Shannon works with younger people to older adults, across NDIS and Home Care Package. She has further training in Pilates and extensive experience with musculoskeletal conditions, physical and neurological disabilities and exercise education and prescription. Shannon utilises a person-centered approach to tailor therapy to achieve the individual’s goals.
Lauri Kirves
Physiotherapist
Lauri works with children, younger people and older adults, across NDIS and Home Care Package. He has experience with rehabilitation, hydrotherapy, physical and neurological disabilities, pain management, exercise education and prescription and assistive technology. He uses a person-centered approach to tailor treatment plans and education to support independence, safety and achieving goals.
Kendall Gleeson
Senior Physiotherapist
Kendall has experience in Pilates, musculoskeletal conditions, rehabilitation, hydrotherapy, post-surgery treatment, pain management, exercise education and prescription. She primarily works with home care package clients to maintain or improve functionality, remain safe and independent in their homes.

Dr Luke Wheeler
Managing Director
Luke started his career as an Exercise Physiologist, then went on to complete his Masters of Physiotherapy, and further trained as a Medical Doctor, achieving honors. He has extensive clinical experience with general medicine, anaesthetics, pain medicine, musculoskeletal therapy, rehabilitation and aged care. When not consulting, Luke provides medical education and professional development for Smart Moves Healthcare therapists.
Pamela Kirgianis
Business Manager
Pamela is a Pharmacist and Credentialled Diabetes Educator with a background in management and business development. She has experience in disability, community and hospital pharmacy, diabetes education and management, mental health and Aboriginal and Torres Strait Islander health. Pamela has experience with non-for-profit and Government organisations, and private practices.
